Dr. Anisha Jacob

Dr. Anisha Jacob is a licensed therapeutic optometrist and is board certified to evaluate glaucoma. She graduated with honors from the University of Illinois in Chicago and received her doctorate in optometry from the Illinois College of Optometry. She completed clinical training with honors at the Williams Chappel Jr. Veterans Hospital, The Atlantic Eye Institute, and the University of Chicago Eye Center. She is a member of the American Optometric Association and Texas Optometric Association. She has been practicing in the Illinois and Texas area  in numerous eye care settings including medical and retail practices.  She offers specialty contact lens fits, low vision exams, and sees children with visual learning differences. Dr. Jacob grew up in Chicago and recently moved to the Dallas area where she now lives with her husband Rony, and sweet German shepherd Saddie. In her free time, she enjoys going on mission trips. Dr. Dannette Lammert

Dr. Dannette Lammert worked as an optometric technician and office manager for ten years, which led her to falling in love with the field of optometry. She grew up in Las Vegas, Nevada and obtained her Bachelor of Science degree in Biotechnology from the University of Las Vegas Nevada. She then attended Illinois College of Optometry and received her Doctorate of Optometry degree. Dr. Lammert completed internships at the VA hospitals in Las Vegas, NV and West Palm Beach, FL where she specialized in a variety of ocular diseases such glaucoma, age-related macular degeneration, diabetic/hypertensive retinopathy, and cataracts. She is licensed therapeutic optometrist and is board certified to evaluate glaucoma. She has practiced in Illinois, Florida, Nevada, and Hawaii in numerous eye care settings including medical and retail practices.

Dr. Lammert practices medical-based optometry and believes the visual system is an integral part of our health to be examined holistically. She provides comprehensive eye care for patients of all ages along with treating and managing dry eyes, red eyes, anterior/posterior ocular diseases and contact lens evaluations. She strives to provide excellent care to all her patients and is excited to take care of you and your family.

Dr. Lammert moved to the great state of Texas over two years ago, first in Austin and recently to Fort Worth to be closer to family. In her spare time, she enjoys hiking, traveling, eating at different restaurants, and spending time with family and friends.

Dr. Nhu Lan Phan

Dr. Nhu Lan Phan was born and raised in Andrews, Texas. She graduated from the University of North Texas magna cum laude with a Bachelor of Science degree in Biology. She attended the University of Houston College of Optometry, where she received her Doctorate of Optometry in 2015. Dr. Phan completed clinical training that focused on ocular disease at Community Eye Clinic in Fort Worth, Texas and Northeastern State University’s Rural Eye Program in Tahlequah, Oklahoma, which served the local Cherokee nation. Dr. Phan is a licensed therapeutic optometrist and glaucoma specialist and is a member of the American Optometric Association and Texas Optometric Association.

Dr. Phan has extensive medical training in the treatment and management of ocular diseases including uveitis, glaucoma, macular degeneration, diabetic retinopathy, corneal disorders/injuries, and co-managements of cataract and refractive surgeries. Dr. Phan practices medical-based optometry and believes that annual comprehensive eye examinations are crucial to the early detection and treatment of systemic and ocular diseases. Dr. Phan’s goal is to provide individualized, up-to-date, high quality eye care to each and every patient.

Dr. Phan practiced in Andrews for 5 years and relocated to the DFW metroplex January 2021. In her free time, Dr. Phan enjoys spending time with family and friends, listening to podcasts, watching movies and documentaries, traveling, and checking out the local craft beer scene.

Dr. Diana Trinh

Dr. Diana Trinh attended Austin Peay State University in Tennessee for her undergraduate study and graduated with her Bachelor of Science majoring in Biology. She then proudly attended the University of Houston College of Optometry and graduated magna cum laude with her Doctor of Optometry degree.

Dr. Trinh completed additional training at the Naval Medical Center in San Diego, California and at the Raymond G. Murphy VA Medical Center in Albuquerque, New Mexico where she specialized in treatment and management of ocular diseases such as glaucoma, diabetic retinopathy, and macular degeneration. Dr. Trinh is passionate about managing not only the eyes but the patient as a whole through thorough examinations and patient education.

Dr. Trinh is a member of the Texas Optometric Association and a member of the American Optometric Association. She is a licensed therapeutic optometrist and glaucoma specialist. Dr. Trinh is avid about traveling and has backpacked solo through Europe and Africa. In her free time, she enjoys trying new cuisines and restaurants around Dallas.
